- Follow up on referrals received from marketing representatives and referral sources.
- Manage the intake process for patients seeking home care services.
- Coordinate and follow up on patient approvals with insurance companies, and other vendors.
- Communicate regularly with patients, family members, referral sources, physicians, and outside vendors.
- Maintain accurate and up-to-date patient records and intake documentation.
- Ensure all required documentation is collected and submitted in a timely manner.
- Monitor referral status and provide updates to patients and referral sources.
- Assist with staffing and case coordination when needed.
- Work closely with intake, coordination, and management teams to ensure timely service initiation.
- Meet departmental goals and performance expectations.
Qualifications
- Strong organizational and multitasking abilities.
- Must have at least 1 year of home care experience
- Spanish speaking is a plus
- Excellent verbal and written communication skills.
- Ability to work effectively in a fast-paced environment.
- Strong attention to detail and follow-through.
- Ability to prioritize tasks and manage competing deadlines.
- Team-player mentality with a positive attitude.
- Ability to handle pressure and adapt to changing priorities.
- Previous home care, healthcare, intake, coordination, customer service etc
